To enroll in the child care referral service print out the appropriate enrollment form. Complete the form and return to Quality Care for Children along with a copy of your Bright From the Start registration. You may fax or mail the form in.

It takes approximately one week to enroll you in the provider database. You are encouraged to enroll even if you do not have vacancies at this time. Please update your records when you have any changes in your program or at a minimum every six months.
