QCC FY 2010 Policies & Procedures Training Assessment
For your convenience, we have posted QCC’s FY 2010 Policies & Procedures Training Assessment.
If you are eligible and choosing to complete the in-home training assessment, you will be receiving by mail the FFY 2010 Policies & Procedures (blue-colored) and the FFY 2010 Rules & Regulations Checklist.
Instructions: Please review these new Policies & Procedures and go ahead and complete the FFY 2010 Rules & Regulations Checklist. Mail the original to QCC, along with the completed Training Assessment.
You may choose to either complete the Training Assessment “on-line” or wait for it to be delivered by mail. From that date, you have 15 days to complete this multiple-choice questionnaire and return to QCC, along with the original, completed FFY 2010 Rules & Regulations Checklist. Please remember to sign and date these documents. Your signature confirms completion of this training requirement.
- Should you choose to complete the “on-line” Training Assessment, you may print, sign, date, scan and email the Assessment and Checklist to jean.stephenson@qualitycareforchildren.org, or
- You may print the Assessment, sign, date and mail to QCC using one of the large white envelopes received from your area coordinator. Whether you complete the “on-line” Assessment or use the one you received by mail, please make a copy of the Training Assessment for your records and keep in your notebook. The FFY 2010 Rules & Regulations Checklist already has a copy included for you to keep in the IMPORTANT PAPERS section of your notebook.
Upon receipt of your Training Assessment and Rules & Regulations Checklist, QCC will review for accuracy and mail you a certificate of completion, which will confirm your FFY 2010 CACFP training requirement has been met.
